2010 Day Camp Forms

Day Camp Info: This is the primary document explaining all the details of Day Camp.

Camper Registration: This is the form needed to register a Cub Scout to attend Day Camp. Each scout must submit this form and a medical form to be turned in by a pack leader at the March Round Table meeting.

Junior Camper Registration: Junior Campers are the younger children of adult Day Camp volunteers who must be cared for while their parents help at Day Camp. They do not participate in the Day Camp program, but a specially assigned group of volunteers spends the week caring for these campers on site. This form is used to register a younger child as a Junior Camper.

Pack Tally Sheet: This document is used by each pack's Day Camp POC to register their pack's scouts for Day Camp. The deadline for submitting this form is the March Round Table Meeting.

Staff Registration: This is the form to register as an adult Day Camp volunteer. A medical form must also be submitted.

Den Chief Info: Den Chiefs are Boy Scouts who volunteer at day camps as assistants to the adult den leaders. They help with all aspects of the daily camp routine and their time counts toward school service hour requirements.  Update June 20, 2010:  All Den Chief training sessions are complete.  There will be no more training sessions this year.

Den Chief Registration: This is the form needed to register as a Den Chief. A medical form must also be submitted.

Medical Forms: ALL participants MUST submit a completed medical form - Sections A and C only. We will be using BSA's new Annual Health and Medical form for all participants regardless of age.  This form replaces the Class 1, 2, and 3 Medical Form.
